ABSTRACT

Hernia and abdominal wall surgery is a relatively new subspecialty in surgery. Although it started late in China, after 25 years of rapid development, it has made remarkable achievements and has become an important part of surgery, laying a solid foundation for the further development of the discipline. At the same time, one should also be soberly aware of the present deficiencies in this field. The development of the field should be more detailed and in-depth from the following aspects: correct understanding of new concepts of hernia and abdominal wall surgery, establishment of hernia patient registration and quality control system, technological innovation and development of technical equipment, especially the expansion of robot surgical systems, materials science progression to hernia and abdominal wall surgery. Faced with this challenge, China is expected to achieve high-quality development in the field of hernia and abdominal surgery.

ABSTRACT

This study combined the herbal pair Platycodonis Radix-Curcumae Rhizoma(PR-CR) possessing an inhibitory effect on tumor cell proliferation and metastasis with the active component of traditional Chinese medicine(TCM) silibinin-loaded nanoparticles(NPs) with a regulatory effect on tumor microenvironment based on the joint effect on tumor cells and tumor microenvironment to inhi-bit cell metastasis. The effects of PR-CR on the cellular uptake of NPs and in vitro inhibition against breast cancer proliferation and metastasis were investigated to provide an experimental basis for improving nanoparticle absorption and enhancing therapeutic effects. Silibinin-loaded lipid-polymer nanoparticles(LPNs) were prepared by the nanoprecipitation method and characterized by transmission electron microscopy. The NPs were spherical or quasi-spherical in shape with obvious core-shell structure. The mean particle size was 107.4 nm, Zeta potential was-27.53 mV. The cellular uptake assay was performed by in vitro Caco-2/E12 coculture cell model and confocal laser scanning microscopy(CLSM), and the results indicated that PR-CR could promote the uptake of NPs. Further, in situ intestinal absorption assay by the CLSM vertical scanning approach showed that PR-CR could promote the absorption of NPs in the enterocytes of mice. The inhibitory effect of NPs on the proliferation and migration of 4T1 cells was analyzed using 4T1 breast cancer cells and co-cultured 4T1/WML2 cells, respectively. The results of the CCK8 assay showed that PR-CR-containing NPs could enhance the inhibition against the proliferation of 4T1 breast cancer cells. The wound healing assay indicated that PR-CR-containing NPs enhanced the inhibition against the migration of 4T1 breast cancer cells. This study enriches the research on oral absorption of TCM NPs and also provides a new idea for utilizing the advantages of TCM to inhibit breast cancer metastasis.

ABSTRACT

OBJECTIVE@#To study the clinical, imaging, and pathological features of pulmonary lymphoma.@*METHODS@#Patients with pulmonary lymphoma diagnosed by lung biopsy in Zhongda Hospital Affiliated to Southeast University from November 2013 to December 2020 were collected and divided into secondary pulmonary lymphoma (SPL) group and primary pulmonary lymphoma (PPL) group according to the primary site of lymphoma. The clinical characteristics, stages, imaging features, diagnostic methods and pathological types of the two groups were analyzed.@*RESULTS@#A total of 22 patients were included, 10 cases were PPL and 12 cases were SPL. The main symptoms of the two groups were cough, dyspnea and chest pain. The proportion of stage III/IV patients and international prognostic index (IPI) in SPL group were significantly higher than those in PPL group (P<0.05). Chest high-resolution computed tomography (HRCT) mainly showed masses, nodules and consolidation in both groups. The proportions of single mass and air bronchial sign in PPL group were significantly higher than those in SPL group, while the proportions of multiple nodules, mediastinal/hilar lymphadenopathy and pleural effusion were significantly lower (P<0.05). The max standardized uptake value (SUVmax), peak standardized uptake value (SUVpeak), total lesion glycolysis (TLG) and metabolic tumor volume (MTV) of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) in PPL group were lower than those in SPL group, but the differences were not statistically significant (P>0.05). In PPL group, 8 cases were diagnosed by transbronchial lung biopsy (TBLB) and 2 cases by percutaneous lung puncture. In SPL group, 4 cases were diagnosed by TBLB, 7 cases by percutaneous lung puncture, and 1 case by surgery. 95.5% patients were diagnosed by non-surgical methods. The main pathological type of PPL was mucosa-associated lymphoid tissue (MALT) lymphoma, while that of SPL was diffuse large B-cell lymphoma (P<0.05).@*CONCLUSION@#The clinical symptoms of pulmonary lymphoma are nonspecific, but the chest HRCT has characteristic manifestations, which can also help to distinguish between SPL and PPL. 18F-FDG PET/CT is also a potential method to distinguish between SPL and PPL. TBLB and percutaneous lung puncture biopsy are reliable methods for the diagnosis of lung lymphoma. The main pathological type of PPL is MALT lymphoma, while that of SPL is diffuse large B-cell lymphoma.

ABSTRACT

Objective: To investigate the relationship between hemoglobin and serum uric acid in adults with various glucose metabolism status. Methods: The demographic data and biochemical indicators of the adult population who had received physical examination in the Second Medical Center of the PLA General Hospital from January 2018 to December 2021 were collected. The subjects were divided into two groups according to the level of serum uric acid: the normal uric acid group and the hyperuricemia group. The relationship between hemoglobin (stratified into four levels of Q1 to Q4 by the quartile) and serum uric acid was quantified by using Pearson correlation and logistic regression analysis. The effects of age and glucose metabolism status on the relationship between hemoglobin and serum uric acid were analyzed. Results: A total of 33 183 adults were enrolled with age (50.6±10.0) years. The level of hemoglobin in the normal uric acid group (142.61±14.24) g/L was significantly lower than that in the hyperuricemia group [(151.79±11.24) g/L, P<0.001]. Univariate Pearson correlation analysis showed that hemoglobin was positively associated with serum uric acid (r=0.444, P<0.001). After adjusting for related confounding factors, multivariate logistic regression analysis showed that hemoglobin was associated with serum uric acid, and the OR values (95%CI) of hemoglobin Q2 to Q4 group were 1.29 (1.13-1.48), 1.42 (1.24-1.62) and 1.51 (1.32-1.72), respectively (Ptrend<0.001) when compared with hemoglobin Q1 group. Subgroup analysis and hierarchical interaction analysis suggested that with the increase of hemoglobin, the serum uric acid in the age<60 years subgroup, normal glucose subgroup and prediabetes subgroup increased gradually (Ptrend<0.05 and Pinteraction<0.001). Conclusion: The association between hemoglobin and serum uric acid in adults is affected by age and glucose metabolism status.

ABSTRACT

OBJECTIVE To analyze the dose-adjusted concentrations of Posaconazole oral suspension in patients undergoing hematopoietic stem cell transplantation (HSCT) and their influential factors. METHODS Data were collected from hospitalized HSCT patients admitted to the First Affiliated Hospital of Shandong First Medical University (Shandong Provincial Qianfoshan Hospital) from January 2021 to April whtwhm@yeah.net 2023 who took Posaconazole oral suspension for the prevention of invasive fungal disease (IFD) and received blood concentration of posaconazole. The rate of concentration attainment and clinical failure rate of posaconazole for the prevention of IFD were evaluated, and one-way and multiple linear regression analyses were performed for the influential factors of dose-adjusted concentrations (C0/D) of posaconazole. RESULTS A total of 44 patients were enrolled; the mean C0 of posaconazole in patients was (0.99±0.94) µg/mL, and 20 patients had a C0≥0.7 μg/mL, with a concentration attainment rate of 45.45% for the prevention of IFD; 13 cases were clinical failures, with a clinical failure rate of 29.55%. Of 24 patients who did not achieve C0/D of posaconazole for IFD prophylaxis, one patient was a clinical failure despite timely dose adjustment of posaconazole in seven patients; seven of the thirteen patients who did not undergo dose adjustment were clinical failures; and the remaining four patients were switched to other antifungal agents. The results of univariate analysis showed that gender, body mass index (BMI), renal function, combined use of sodium phenytoin, omeprazole and metoclopramide had a significant effect on the C0/D of posaconazole (P<0.05); the results of multivariate linear regression analysis showed that gender, BMI and combined use of sodium phenytoin were the independent factors affecting the C0/D of posaconazole (P<0.05). CONCLUSIONS Significant individual differences are reflected in the blood concentration of Posaconazole oral suspension; gender, BMI and combined use of sodium phenytoin are independent factors affecting the C0/D of posaconazole.

ABSTRACT

ObjectiveTo screen the appropriate reference genes for real-time fluorescence-based quantitative polymerase chain reaction(Real-time PCR)analysis of the Andrographis paniculata under methyl jasmonate(MeJA)and various abiotic stresses. MethodThe actin 1(ACT1),actin 2(ACT2),elongation factor(EF-1α),glyceraldehyde-3-phosphate dehydrogenase(GAPDH),tubulin(TUB),polyubiquitin(UBQ), and 18S rRNA(18S)gene were selected as candidate reference genes based on the RNA-seq data of high temperature, drought, UV, and MeJA. The expression of seven candidate reference genes in the A. paniculata leaves was assessed by Real-time PCR,and the stability was analyzed by geNorm,NormFinder,BestKeeper, and Refinder. ResultThe results of stability evaluated by geNorm,NormFinder, and BestKeeper were not the same due to different indicators. As analyzed by Refinder, for the stability of the expression, the genes were ranked as UBQ>18S>EF-1α>ACT2>ACT1>GAPDH>TUB under high temperature stress, ACT1>UBQ>EF-1α>18S>ACT2>GAPDH>TUB under drought stress, EF-1α>TUB>ACT2>UBQ>18S>GAPDH>ACT1 under UV stress, and ACT1>EF-1α>UBQ>ACT2>18S>TUB>GAPDH under MeJA stress. Among them,18S gene was not suitable as an internal reference gene duo to its high expressive abundance. This study also verified the relative expression level of andrographolide synthesis-related gene hydroxy-methylglutaryl-CoA synthase (HMGS) in the four stresses on the basis of transcriptome data, and found that the Real-time PCR results of appropriate internal reference genes were accurate and reliable. ConclusionUBQ-ACT1-UBQ,EF-1α-TUB,and ACT1-EF-1α were the suitable combinations under stresses of high temperature, drought,UV, and MeJA. This study is expected to provide references for the research on function regulation and expression of genes in A. paniculata under high temperature, drought,UV, and MeJA stresses.

ABSTRACT

The plant growth, development, and secondary metabolism are regulated by R2 R3-MYB transcription factors. This study identified the R2 R3-MYB genes in the genome of Andrographis paniculata and analyzed the chromosomal localization, gene structure, and conserved domains, phylogenetic relationship, and promoter cis-acting elements of these R2 R3-MYB genes. Moreover, the gene expression profiles of R2 R3-MYB genes under abiotic stress and hormone treatments were generated by RNA-seq and validated by qRT-PCR. The results showed that A. paniculata contained 73 R2 R3-MYB genes on 21 chromosomes. These members belonged to 34 subfamilies, 19 of which could be classified into the known subfamilies in Arabidopsis thaliana. The 73 R2 R3-MYB members included 36 acidic proteins and 37 basic proteins, with the lengths of 148-887 aa. The domains, motifs, and gene structures of R2 R3-MYBs in A. paniculata were conserved. The promoter regions of these genes contains a variety of cis-acting elements related to the responses to environmental factors and plant hormones including light, ABA, MeJA, and drought. Based on the similarity of functions of R2 R3-MYBs in the same subfamily and the transcription profiles, ApMYB13/21/35/67/73(S22) may regulate drought stress through ABA pathway; ApMYB20(S11) and ApMYB55(S2) may play a role in the response of A. paniculata to high temperature and UV-C stress; ApMYB5(S7) and ApMYB33(S20) may affect the accumulation of andrographolide by regulating the expression of key enzymes in the MEP pathway. This study provides theoretical reference for further research on the functions of R2 R3-MYB genes in A. paniculata and breeding of A. paniculata varieties with high andrographolide content.

ABSTRACT

Objective:To explore high-resolution MRI (HR-MRI) characteristics in patients with ischemic stroke caused by vertebral artery dissection. Methods:A total of 47 patients with suspected vertebral artery dissection in the First People′s Hospital of Lianyungang, Kangda College, Nanjing Medical University from June 2015 to June 2020 were consecutively enrolled. All patients underwent routine MRI before HR-MRI, and three-dimensional arterial spin labeling (3D-ASL) was performed in those with negative MR. Patients with posterior circulation infarction on MRI and ischemic hypoperfusion on 3D-ASL were included in the ischemic stroke group, while patients with negative plain MRI and normal 3D-ASL were included in the normal group. The clinical characteristics and HR-MRI imaging characteristics between the two groups were compared using t-test or χ 2 test, while the correlations of quantitative data or ranked data were analyzed by Pearson or Spearman test. Results:There were 27 patients in the ischemic stroke group and 20 patients in the normal group, and there were no significant differences in clinical characteristics such as gender, age, site of onset, history of hypertension, hyperlipidemia, and history of diabetes between the two groups ( P>0.05). Basilar artery lateral deviation classification (χ2 =7.013, P=0.030), basilar lateral bend angle (140°±19° vs. 137°±15°, t=2.231, P<0.026), minimum bending angle of vertebral basilar artery (131°±27° vs. 90°±13°, t=42.630, P<0.001), the minimum bending angle of vertebrobasilar artery ≤ 90° distribution (3 cases vs. 21 cases, χ2=15.240, P<0.001) and effective lumen index (0.33±0.10 vs. 0.17±0.09, t=35.934, P<0.001) of normal group and ischemic stroke group showed statistically significant differences. Among them, the minimum bending angle of the vertebrobasilar artery was negatively correlated with posterior circulation ischemic hypoperfusion ( r=-0.621, P<0.001), and the effective lumen index was negatively correlated with posterior circulation ischemic stroke ( r =-0.713, P<0.001). However, the location of the dissection, the distribution of hematoma, the shape of the lumen and the enhancement type were not statistically significant between the normal group and ischemic stroke group ( P>0.05). Conclusion:HR-MRI shows that the lateral deviation of the basilar artery of grade 3, the minimum bending angle of the vertebrobasilar artery less than 90°, and small effective lumen index are related to posterior circulation stroke caused by vertebral artery dissection, which may help for the proper formulation of clinical treatment plan.

ABSTRACT

OBJECTIVE@#To investigate the correlation between morphological stages of midpalatal suture (MPS) and Demirjian dental age.@*METHODS@#In this retrospective study, 1 076 cone-beam CT (CBCT) images (female: 675, male: 401; age range: 6.0 to 21.0 years) were included. Horizontal view of each sample's CBCT images was observed, each sample's MPS stage was recorded, and dental age. MPS stage and dental age were ordered with categorical variables. Therefore, their correlation was investigated through Spearman correlation coefficient analysis and diagnostic test analysis.@*RESULTS@#(1) For left lower second premolar: 95.2% of those in dental age stage B-D were in MPS 1-2, accounting for the largest proportion. 85.3% of those in dental age stage E were in MPS 1-2, still accounting for the largest proportion. Another 14.7% were in MPS 3.45.1% of those in dental age stage F were in MPS 3, 46.1% in MPS 1-2, and another 8.8% in MPS 4s1.49.8% of those in dental age stage G were in MPS 3.24.9% in MPS 4s1, and 18.9% in MPS 1-2.80.1% of those in dental age stage H were in MPS 4-5. Another 16.3% were in MPS 3. (2) For left lower second molar: 89.7% of those in dental age stage B-D were in MPS 1-2, accounting for the largest proportion. 67.4% of those in dental age stage E were in MPS 1-2, still accounting for the largest proportion. Another 26.1% were in MPS 3.55.3% of those in dental age stage F were in MPS 3, 34.2% in MPS 1-2, and another 10.5% in MPS 4s1.50.7% of those in dental age stage G were in MPS 3, 24.3% in MPS 4s1, and 16.8% in MPS 1-2.83.8% of those in dental age stage H were in MPS 4-5, another 14.2% were in MPS 3. (3) To diagnose MPS stage with dental age, diagnostic pairs with good performance included: Dental age of left lower second molar-MPS: H-4s2, H-5, D-1; Dental age of left lower second premolar-MPS: H-4s2, H-5, G-3. Other diagnostic pairs were of ordinary diagnostic efficiency. (4) For dental age-MPS Spearman correlation analysis, dental age of left lower second molar-MPS had the highest Spearman coefficient (0.68), dental age of left lower second premolar-MPS was the second high (0.64). (5) Dental age stage H of left lower second molar or left lower second premolar indicated that the individual was later than MPS 4s2.@*CONCLUSION@#Dental age's diagnostic efficiency for MPS stage is ordinary on the whole, except for some pairs with good performance. Therefore, pre-treatment CBCT examination should be considered as assistance for evaluating maturation and fusion status of midpalatal suture.

ABSTRACT

Objective:To compare the application value of different polypropylene mesh in inguinal hernia repair of adults. Methods:The prospective cohort study was conducted. The clinical data of 120 adult patients with inguinal hernia who were admitted to two medical centers (60 in Huadong Hospital affiliated to Fudan University and 60 in Shanghai Ninth People′s Hospital affiliated to Shanghai Jiaotong University from March 2012 to Match 2014 were collected. Patients were randomly divided into study group and control group using the random number table. Patients in the study group underwent repair of inguinal hernia using the SMP95958X mesh, and patients in the control group underwent repair of inguinal hernia using the modified Kugel mesh. All patients underwent preperitoneal repair by senior hernia surgery specialists. Observation indicators: (1) postoperative pain; (2) complications and follow-up. Patients were followed up at postoperative 3 months and 6 months using outpatient examination to detect the short-term complications by physical or color doppler ultrasonography examination, and at postoperative 5 years using telephone interview or outpatient examination to detect long-term complications including infection, foreign body sensation and recurrence of hernia. The follow-up was up to March 2019. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was analyzed using the t test. Measurement data with skewed distribution were represented as M (range), comparison between groups was analyzed using the nonparametric rank sum test. Count data were described as absolute numbers or percentages, and comparison between groups was analyzed using the chi-square, continuous correction chi-square test or Fisher exact probability. Comparison of ranked data were analyzed using the nonparametric rank sum test. Results:A total of 118 patients with inguinal hernia were selected for eligibility, including 116 males and 2 females, aged (64±12)years, with a range from 29 to 84 years. Of the 118 patients, 59 were in the study group and 59 were in the control group, respectively. (1) Postoperative pain: of the 59 patients in the control group, 34 took painkiller and 1 case lost the data of taking painkiller at postoperative 2 days. Of the 59 patients in the study group, 29 cases took painkiller. There was no significant difference in taking painkiller between the two groups ( χ2=1.055, P>0.05). The pain score at postoperative 2 days and 3 months were 3.26(range, 0.70-6.90) and 0.87(range, 0.00-4.10) of the control group, respectively, and 3.03(range, 0.00-8.80) and 0.83(range, 0.00-3.10) of the study group, respectively, showing no significant difference between the two groups ( Z=0.782, 0.729, P>0.05). (2) Complications and follow-up: the incidence of postoperative complications at perioperative period (within postoperative 2 days) was 1.7%(1/59) and 1.7%(1/59) in the control group and study group, respectively, showing no significant difference between the two groups ( P>0.05). Both of 59 patients in the control group and study group were followed up for 6 months after operation, respectively. The incidence of postoperative complications at 3 months and 6 months after operation was 1.7%(1/59) and 1.7%(1/59) in the control group, respectively, and 5.1%(3/59) and 5.1%(3/59) in the study group, showing no significant difference between the two groups ( P>0.05). Fifty-five patients of the control group and 52 patients of the study group were followed up for 5 years after operation, respectively. There was 1 case of infection in the control group, with the incidence of postoperative long-term ( within 5 years after operation) complication of 1.8%(1/55), and there were 2 cases of infection and 1 case of foreign body sensation in the study group with the incidence of postoperative long-term (within 5 years after operation) complication of 5.8%(3/52), showing no significant difference between the two group ( P>0.05). There was no recurrence of hernia in either group. Conclusion:Both of the SMP95958X mesh and the modified Kugel mesh can be used in preperitoneal repair of inguinal hernia, showing no significant difference in the efficacy between them.
